I have a form on my website page that is basically a 10 question simple survey with 1 yes or no answer for each question.

I need 5 questions from this form removed.

Thats is, probably is just a simple cutting of a few lines of code, but I dont want to mess up the form.

This job needs to be done immediately upon bid acceptance.

I like the rounded corners and the captcha verification.

The plugin will need a simple admin settings page.

All code must be written by you, do not plagiarize or copy code from anywhere.

need a contact form to send admin html mail exactly like attached image, but with extra field "contact number".

So 4 fields: name, email, contact number, message.

Also, send a "thank you" message to sender.

milar to existing plugins. Once contact form is saved a table will be created just for that form with fields that are the same as contact form fields.

Once the form is submitted the admin can view forms in admin sidebar with little numbers for new submissions. Once admin clicks on form the admin can then view/sort/delete/export form submissions on a per form basis.

d to have a 1 page simple HTML contact form using email-form.com script. We have purchased the script and have installed on the server but need HTML page and contact form created. We have a PDF brochure that contains 90% of the information we want to ask with the online form. The form's option will be to save results to a XLS for spreadsheet and email to multiple emails with thank you page for more contact. All is possible through the installed script. 2. contact form - when you click contact us, a dropdown ajax form is needed.
4 fields with validation: name, email, phone, Message box. all fields except phone are required fields.No database work.
form will send me the message and send a confirmation email to user. also a Thank you message on submit.
