Design Of Leaflet And Form In Indesign

I need a template design of the attached leaflet and form, both are double sided, this must be designed in adobe indesign and also to print standards.
all i want to do after its done is edit the words and pictures ect, the fonts and colours must be very similay if not exact.
And the whole design must be in layer and fully editable with trim marks
and crese mark ect as it would normally be.
This project is quite urgent and must be treated as a priority!
