Integrating Google Maps And Creating Multi Part Form

We have the webpage www.home-rescue.co.uk/home2/index.html which is getting us a great conversion rate of 50%+
However the second page the user lands at does not have a great conversion rate and we are looking at ways to improve the conversion.
This is what we want.
1. The user enters what is called their zip code (post code in UK) on the page above.
2. They get taken to another page which displays a map of their post code and a 5 mile radius. This map will have a pin located at the post code the user has entered.
3. This map will also have other pins located from previous enqurieis we have got in the same area. These other areas will be provided and needs to be added to the map. The pins from these enquiry post codes needs to be a different color.
4. On display of the map, we also need a form preferably multi part form that collects contact details, property details & financial details much like the existing form.
5. The form needs to save all data to a database and email the results.
6. The form should store all the details it has captured even if the user has not completed all three parts of the form.
7. The page needs to be a similar design to what is already in place.
8. All the action items need to be above the fold, so should fit on the screen without the need to scroll.
9. In completion of the form the user needs to be taken to a thankyou page which already exists.
For any postcodes which are invalid and cannot be found on a map, a page needs to be displayed saying that we don't deal with property in this area or something similar.
Knowledge of google maps api, php, mysql needed.
Just 1 page needed to be created and it may require changing the home page above to integrate correctly.
An easy project for a good developer with the necessary skills. Payment on completion, from a good buyer with excellent rating.
