I need this form, attached, retyped, and recreated exactly (I will provide additional logo later) using Adobe InDesign and Acrobat Pro.

does not hide them.
-Generally speaking the form does not seem to work perfectly at all times and we're not sure if it's an acrobat version thing or a pc vs mac platform thing or glitches due to the way it has been set up?
I've attached a copy of the form for your review. the "reveal scores" button is the red dot in the logo at the top of page 1
We need this form to be technically perfect and functioning properly regardless of the system or acrobat version.

Small Fillable Pdf Form 2
25 gift certificate. It will ask for their full name, full mailing address, telephone and email address. It also will ask for permission to contact them for specials and promotions. I don't want the bid to be no more than $ 5 because this is a small project.

Auto-populate Pdf Form Fields Based On User Choices
I will need to be provided all scripting and/or table format used in order to accomplish this project so if any changes need to be made in the future I have all the information.
I have attached the orignal blank form.
Any finished product (auto-populating form) will be my property and no further distribution of this form will be allowed. Once the bid is accepted I will provide my custom form of this pdf so that the auto-populate features may be added.

Import Xml File To Pdf Form
this is very simple. BUT you need to have Adobe Acrobat Pro in order to do this task. I just need you to take my PDF form, open it in Acrobat Pro, then go to the Forms menu and click Import Data. You will import the data from an XML file that I send to you. I would do this myself, but I don't have Acrobat Pro. I need this done now!

- We'll need to modify some of the texts ourselves (conditions of the contract at the bottom) so I guess that with Adobe Acrobat pro we have, we can edit that part easily at any time if we want.

We need to create exact copy of the (attached here) fillable pdf form but in html. The users must be able to fill exactly the same fields as they do in the attached pdf form. When they hit "send" the filled form must be sent to email address as attachment and should look exactly like the attached pdf form but filled with user inputed data.

PDF interactive PDF fill in areas
I have an interactive PDF where my customer fill in blank spaces. The design needs to use the same files, but refreshed. I have a new logo to put on the PDF. The form fields won't change much. 1-3 pages. I will provide the original document. Blank areas need to be large enough to hand write information in the blank areas.

We looked online for a template, but nothing matches perfectly, so please do not bid unless you can create the form.
Attached are a few sample style documents, but what we want is different. The sample are juat intended to show you that the design, color scheme, layout, and functionality are very important.
