Convert Word Document Form to HTML Form.

We want to convert the attached Word Document form to a HTML form. When the form is submitted it will need to save the form content as a PDF file and then either save it to the server or email it.
The HTML form should be designed neatly as should the output PDF.
The form should have live client-side validation and tips as well as server-side validation.
