Calculator With Rss/csv Input

Ok i need very basic calculator which calculates price based on glass size. Please take a look on screenshoot and xls attached. In xls you will see the types/subtypes of mirrors.
There 4 fields:
Size a
Size b
Type:
Sub type:
Baased on selection of subtype it will be calculated the price of mirror for that size.
There will be also 2 hidden inputs which will be passed by me to php cart. I will do that on my own. You just need to create calculator and get a better way to include all that prices: maybe rss? for later edit.
I have already created the html code for calculator base so you can check and use too.

Ajax/web 2.0 Calculator
Ajax pro needed to create a multi-step calculator with many input fields. The calculator will require several expanded or pop-up boxes for more information and/or input fields. Calculator will be plugged into wordpress page(s).

Small site that with DB input/output
