We require an online database that allows us to design our own forms and databases online for telesales staff for multiple campaigns
There will be an option to allow us to select from a variety of predifined common fields (like name, telephone number etc etc), and an option to also input unique fields
The forms will be there to allow us to capture data on the telphone and then export for that campaign.
there will also be an option to allow us to import data into a campaign (ie list of names and numbers).
Some fields will be text and some will allow us to open up other sections of the form (ie if a question "are you a homeowner?" had yes/no option, and yes was selected, then this would open up other fields to fill in).
the system will allow telesales users to log into the system and select the campaign that they are working on. the system will then track what records they have inputed.
we will also require the option to be able to put promts/sctipts or questions onto the forms (ie for things like "hi this is ________ calling from abc company..."

1) One field needs a drop-down box with 6 Options to choose from.
2) If one of the options in 1) above is selected then a number must be inserted in another existing field on the form.
3) We have three other options to choose from. If the user does not select one of the radio buttons then the user must be informed to select an option before re-submitting the online form.
4) One of the fields must only allow 6 or 13 characters.

ere must be a reports function that allows admin to retrieve the CNA's form for the week. The database will also archive the form data. Data retrieved must populate the online form for online viewing of each patients record that is stored. The CNA's must log in to create, view, modify and submit completed forms.
Only admin will be allowed to run reports. One of the reports should only display time totals for the CNA's for the week rounded to the nearest 5 minutes.
