Simple Script / Creates Folders + Custom Index.php In Them

I need a simple script that can do the following:
1. It askes me to fill out "main_folder_name"
2. Askes me to upload CSV file. The CSV file will have 2 sets of variables. First variable is "folder_name", and the 2nd one is id value.
3. One must click a button to make the script work
When clicked:
3.1. The script generates X ammount of folders under main folder name. The folders are created depending on the folder names given on the CSV.
3.2. Each folder name includes a custom index.php (it has 5 lines).
index.php must include 2 changes in the code. Main_folder_name and id
This script can be run on Linux/PHP. I dont need it to be a web script, if you can run it into Windows interface, as a program, better.
I would prefer the output to be stored in a ZIP file. That way I can upload the files in the proper places.

Anyway - that is the basic concept. As I said before - it should be a very simple script.
If you can do it as a drupal module, that would be awesome - but not required. It needs to be able to work with our shopping cart so if our members credit card fails, it can post back the info to your script and lock out their account. If it's in drupal then we have this code already done.

Since I will use this under wordpress, if you can make it as a wordpress plugin, it will be even better.

I had a script created that can search craigslist for records and outpout them and it has all of the sudden stopped working. Some of the values are not being displayed. This is very simple. Complete the code, show me it's working and I'll select you and pay you. The code is provided in the PMB. All I need you to do is tell me what lines I need to change via PMB. Then when clicked again to play, it hides the text again.
Should be extremely simple for someone who knows what they are doing.
