Quick Updates To Online Survey

I have an online survey that was recently created using PHP/JQuery/MSSQL. I have a couple of quick additions I need made to the survey.
The survey has 5 possible responses for each question. Currently, if the end-user answers with a 1-4 a "Comments" box pops up for them to enter comments. I also need it to display another question upon popping up the comments box.
Also, it's not recording to the DB right now. It was until I made a few minor edits to the text, so I obviously am missing a semicolon or something small like that.
This is a very very quick project. I need it done right-away. I also need you available via instant messenger during the project. Please only bid your best quote if you can work begin working on this immediately and have it completed within the next hour or so.
I will post a working version of the survey to the PMB upon request.

Simple And Quick Updates Flash Template

I will provide a playground / test site for you on my server with FTP access to the files if you wish. I will also provide a simple specification to make things a little more clear.

because sometimes there can be 20 questions we do not want to show all on one page so it should be possible to divide question based on category into many page so when user filled first page he press next etc..

On the screenshot, I have highlighted (in yellow) the fields that need to be customizable in the plugin "settings" page. (I have labelled each separate field with an "F"; so "F1" is field 1; "F2" is field 2; etc.)
