Php Form W/ Dynamic Thank You Page

I need to create a php coupon request form. A person will fill out the form to request a coupon. Once they hit submit they will be taken to a thank you page that will display the coupon. The coupon that displays will correspond with a question in the form (there are 5 different coupons). In addition I need some text to overlay the coupon image. This text will be created based on form information. I also need the coupon thank you page to display a voided coupon if a person goes to the page without filling out the form. To sum up, the form needs to do the following (see attached):
1. Collect basic info, including what coupon they want
2. Verify information
3. Form submitted
4. Display coupon with text overlay (for printing)
5. Send form information to me via email
6. Display a voided coupon if the form is not filled out

I desire a holiday sale project of digital products. I want two sales pages and a thank you page with download links. I will provide domain/subdomain name and hosting location, images, more info link,download links, banner code and opt in box code. I will require security added to the thank you page. I want a well know Christmas song to play when page one and two are opened.

Setting up my aweber to pass info to a thank you page
er email system. I would like to set up the page so that it will pass the email info from the squeeze page to the thank you page where it will autopopulate another form. Aweber has an option to do this, as well as my affiliate program has an option to receive this information, I just can't figure out how to set it up. Your help would be appreciated. Thanks.
