Clean Up Perl Cgi Form Page

If you write CGI perl scripts and can modify an existing form, you are our programmer.
We have a basic html index page with a form at the bottom of the page. The form asks for name email and some other misc information. When the form is submitted it goes to a cgi perl script that sends it to our contact management program.
We need the form cleaned up so that:
1. All fields that are requested to be completed, are in fact completed. If the site visitor does not complete the field it should prompt them to do so.
2. Three of the fields need to be in proper case. Meaning if someone types in JOHN SMITH, then the result is John Smith into our database. Or if they type john smith, then it converts to John Smith. Basically the first letter is always a capital and the rest lower case.
3. They are prompted for their email address. We need to make sure the email address is as clean as possible coming into the system, meaning it contains an "at" sign and a "." and no spaces.
4. We have two fields for the visitor's email. One requesting their email address and the second to confirm it. We need code in the form that requires these fields to match exactly. AND the second email field confirming their email should serve as their "account number" with us so a COOKIE should be created for their system that identifies their email account number on future form pages. This way they do not have to type it in over and over again on other pages.
5. Final item is the return/results page. Currently the results of the form submission after being processed by the cgi perl script it sends the visitor to another page. This code is currently in the cgi script. I would like it to be in the form page and not the cgi script. This way we can direct the visitor to what ever page we want without modifying the cgi perl script.

Perl Cgi Repair

If we need to build a new form with the same fields we can. I want to get this done today as I know it should not be a huge project.

Fix Cgi Form Script
I need a CGI person to trouble shoot and then fix why we are not getting the emails that contain names of people who have downloaded a report from our site. The script is CGI. The downloads work. People complete the form and the PDF document hits the screen. We are just not getting the names forwarded to us by email. The form email generation had worked until now.

Photos need to be resized, moved to image folder, and re-linked
Some nested tag issues causing margin probems
Tag problem causing bottom of page to show up at top and some other tag related formatting issues.
Just clean it up and make it look good.

Perl/CGI script repair
We have changed the server for the websites of one of our clients. Since than some of the CGI scripts are not working anymore. We have given the job to one of the GAF members here but he made the situation worse and big part of the pages are offline now.
