Remove the spaces in the credit card number in the email sent to CanLaw when people place an order.
Look at the order form at https://www.canlaw.com/secure/forms/orderformdl.htm
The form is set up so people enter credit card numbers 4 digits at a time in each of 4 boxes. I want that to remain as is. People enter in this format xxxx xxxx xxxx xxxx
CanLaw gets that order as xxxx, xxxx, xxxx, xxxx
We want to get it as xxxxxxxxxxxxxxxx
HOWEVER, when the email is sent to CanLaw, we want the credit card number to show as 16 digits with no spaces (15 digits for AMEX)
This is a minor matter for us, but it would save us manually removing the spaces when we process the orders.
You will probably need our formmail script to make this change. Let me know

Multi-part Service Order Form

An order confirmation should appear on the next page with their order details and they should also be sent a copy of the order to their email address (if possible).
Since I am integrating this with a Wordpress site, I would like to just simply have the ordering link open in a new pop up window that takes up about 1/2 of their screen, so they can still see our website. So it does not need to be integrated with Wordpress, HTML is fine.

a very simple credit card application on my site that can be downloaded by customers as a PDF. The must fill it out and Fax it back.
I want to make it more user friendly. I want it to look like this: https://www.fleetcardsusa.com/apply3.aspx?productID=60#. It should be a four step process just like this site but customized to my site, not an exact replica. All information from form must be sent to my company email address.

Promo code option needed for an existing order form, when the correct code is entered a percentage will be taken from the existing auto total calculation on the forms. Uses JS and php, MUST work with both IE and firefox. if wrong code is entered then the user needs to be notified. No databases involved!
