There would be an admin page with check boxes next to each field, if checked it would be displayed on the contact form.

Features:

- Need a hook to allow other plugins or theme functions.php to add new fields (see wordpress codex for do_action and add_action)

- Save settings to wordpress database options

NOTES: I just need the field selection part of the script, I will apply this to my contact us plugin.

2 -I have installed 'Simplemap' plugin which seems to be a great store locater plugin. I would like this plugin to read from the API and set up each account holder as a location, and assign the product lines they carry as catergories
