Print Mailing Labels

I need a custom script written.
when run it will ask for a date. (maybe have a nice date thing come up to select the date with)
after the user selects a date and hits submit, the script will goto
http://www.pbso.org/index.cfm?fa=blotter
and enter the date there so the data comes up....(I do not need to see the data)
Then create a PDF or word doc... with only the name and address of the people on the report.... format the name and address to fit on mailing labels... we use mailing label "avery 5160" then we hit print and the labels will print correclty on our printer...
that is it....

I need someone to take a word doc file with appx 400 names and addresses and print mailing labels. Then mail the labels to me.

Update Fedex Print Labels On Cre Loaded From Oscommerce
bers are included in the email that is sent to the customer when the status is updated.
All we need is the Print Labels updated. We ship everything for free so no shipping prices need to be sent to the customers when they order. We just need to be able to print the labels so we have them to put on the boxes for shipping.
We already have all the fedex testing acount info setup.

Create Avery Labels From Mysql
clients I want to print the adress. So I have to slect them first.
