Php/mysql Form Help

I am in need of an experienced developer with PHP skills to help me finish a form for my websites launch.

I have the form marked up in XHTML and have a PHP template, I have just never used PHP and need help writing the final code.

Looking to finish this by this weekend.

Hi, I need some help to convert static page form to joomla, its a litte difficult so please only bid if you know how to do it.

nd paste form stuff I'm sure, I just dont have the knowledge to tweak the forms that are out there to get them working on my websites. I've been screwing around with this task all day and cant get it working. I just need you to get the form code and corresponding asp and php file created and working together on one page of each respective site such that I can cut and paste the form code into the few other pages I will have.

I need a contact form popup. I want it to look like this one. http://form.jotform.com/form/13524926247 I DO NOT want the big white highlight. I want all fields to be required. And i want to be able to edit it easily(email). I want an close button on the top right that looks like this. http://www.weloveschoolrunmums.co.uk/images/popClose.gif but i want the red to be black. Please help.

the checkboxes, that's fine.

I've got all of the work done ... But the problem is that the php handler is not allowing the form to be submitted unless at least one checkbox is selected.

Also, I have a custom "submit" button, but I'm having trouble getting the php handler to accept and properly handle the form with it.

Can anyone help??

Together both scripts work ok accept for the onblur effect. the jqtransform doesn't change the backgrund image back to the previous state.

I need to try to get these 2 scripts working together or another text box hint script to work with it.

I will handle the database insertion. This will mean creating a simple php page that echos the variables for testing purposes.

I may require some help troubleshooting when I add the form to my main site just incase there is any conflicts with javascript files im already using in the body.

I need someone to help me submitting a ajax form with prototype. I have the form working,but it gives a javascript error when trying to post values from radio or checkboxes. This should be an easy fix for you, so do not bid over the max amonut. I need someone to help right away.

On clicking submit the visitor is redirected to Nochex to make payment, while also having the data emailed to the site owner.

Currently the forms are setup but aren't integrated properly together.
The script used is formmailer. I would like this setup without incorporating any javascript.

I have a fully working website with one simple email form using MailForm.pl. It worked fine until we transfered from one hosting company to GoDaddy.com.
I get a 500 Internal Server Error when the form is submitted now.
I would assume I only need a hour of help or so, but I can not get this form to work now.

I need help in php form code. This page was working until its moved to new location. Php form is suppose to insert data in mysql. But its not working. Can someone good in php and mysql figure this out. I need this fixed soon. I can pay upto $ 50 if this code can get fixed today or by tomorrow.

The second form is on the same page and this should be pretty easy to solve. I want the user to be able to use the drop down to secect their quantity. Then, the form needs to format a url using that selected number and open a new window.

I need help with this form. It is written in html for easy editing. This should be easy for anyone. i want someone to choose from a product in each category (yellow,red,green) then when they hit submit it searches the website using the the link on the top. Each product has a radio button and only one product can be chosen from each column. the radio button that was chosen then is place in the link as shown in yellow,red,green. Best price and best work gets the job. thanks
