I need a form where the visitor can do a search on the page by clicking on a map of the state of usa then it will list a map of that state then they can select the city. Once they have selected the city it will display the content.
There needs to be a control panel where I the seller can log in and post there ad to the specified state, city and the form will bill them through paypal. Once the payment is completed the form will post there ad in the specified state, city. Payment needs to be set to bill monthly though paypal.
The seller should be able to log into there control panel and view there ad and cancel or change there ad.
Form needs to be tested on your server before delivered to me.
Project needs to be completed and delivered within 1 week of acceptance. If project is not delivered in the time frame, the programmer agrees to cancel and refund project fees.

The main point of the excersie is to make the form look EXACTLY how I have the current dummy form created in HTML

i have a site www.frboclassified.com and i need a more detailed search form on it, that will search country, state, city, type of property, # of bedrooms, # of bathrooms, price range etc.... i need this as soon as possible...
Need an online form created that feeds a document
reated that is a series of questions that someone answers that then combines with each other and produces a Pdf and word document. There will be dropdown selections as well as type answers. In addition there are numbers that they need to enter that will need to then uses formulas to create tables. All these formulas and tables are created in excel already and the questions are written out and seperated by sections to form the template.

Search Form Script Php Or Suggested
I am looking for a script where if someone uses the search form on a certain keyword if listed on the list would add another keyword next to it to help refine the results on known errors to show a more relative listing.
For example if a user searches mask and presses enter/or clicks on the search button. It would scan the list of keywords and if there for "mask" if identical 100% to match would change the search to "Latex+Mask" at the submit.

I need 6 forms created. on a windows server. Currently the form is a PDF file... we need to make it so you can edit the PDF like a form, then it gets emailed to me as a PDF exactly like the form. should be very simple forms to create. nothing complicated
