We need a form with a 8 fields that will capture all the info into a csv file. We also need a cron job setup to email the admin the latest csv file if there has been new entries added to the file. We'll need this in 1 day. We'll provide the design which will be a simple header and then a standard form layout.

I have a simple HTML website form that is designed to capture users for a trial of a service i am offering. I need a Javascript and PHP person to post the form infomation via CSV to an email address.

With javascript, i need to have an add user button that creates a new field entry as required.

The client information is sent to our web server, and the song and image is sent to an email address.
