Clean Up Php Form- Drupal Site

Hi, I have a form that is too difficult for me to manage right now. The form can be found here: http://197bankruptcy.com/node/5
It is very long and the html code is wrong. I need someone to fix the html, convert it to a php form, break it up in to 3 pages, and make it look better with css and stock photos if possible. Also, delete any unnecessary code. I'm looking to spend up to US$ 75 on this project, maybe more if you do an amazing job. I can pay with paypal. The design of the webpage is probably going to change, so keep that in mind when working on this project.

Clean up design on Drupal website of ours

drupal 5 with some custom modules. New there will be based on this template:
http://www.templatemonster.com/drupal-themes/20728.html. I have changed logo images. New template is using phptemplate engine (v 6.3), so there will be work of porting it back to version 5. Also, we have some custom blocks and module which may not be available in the theme, therefore it is not just putting new theme in the folder, developer needs to have good understanding of Drupal APIs, php and mysql.

Php Form - Need Sessions Added
have a PHP form that has been created for me. It has 11 or so steps, and posts to an SQL database.
However, there are no sessions so half way through the form you can't go back and forth.
I need someone to edit this form for me. I'll pick a programmer in the next hour and ideally it would be great if we could get this done in 2 hours or so.
I'm in the UK so I would prefer a UK programmer as the time difference doesn't suit.
