In Need Of A New Website

I am too busy at work to do any of this on my own, so while I'm able to do some of this project, the reality is I'm just going to contract out, and see what it cost.
I own the rights to howtobuywithbadcredit.com. I will need a site designed, including a flash introduction. I can give examples of others sites I like, other intros I like, but I don't have anything specific in mind yet, so design would be partially on your end.
The following details will need to be in the home page.
1a) A professional look
1b) A basic flash intro that involves a spinning/moving logo
1c) The ability for me to put ads/links on the side to other websites
1d) Drop down menu to the rest of the site
The other pages will include an about us, a history of credit page, a reference page listing potential links and most importantly, an application, which will have fields for them to fill out, and some of the fields will have drop downs only allowing them to select from a pre-set list of answers.
The applications will need to be forwarded to my email address as well as a text notification of any applications sent to me.
For the backend, I should be able to edit/add any images on the site, except the flash intro. I should be able to easily edit the application fields.
I will also need the site hosted. In addition, I am considering adding videos to the site (not initially), and would want to know if your hosting ability enabled this.

I am a musician in need of a website that can be fully interactive. I would like for my fans to be able to listen and preview music. Also I would like for them to be able to purchase my music via download. I would also like to have the ability to link my website to any online store that my music can be purchased on.
