Convert My Php Form To Html
I have a form that is written in PHP. I need this form to be converted to HTML since PHP doesn't work properly with my web design tool (iWeb). A picture of the form is attached. I will provide the PHP code to the professional that I choose to work with.

Ok, I'm not that good with php, so I need someone who can write me a php script that will mail a simple form to email. It will need: required fields, success redirect, fail redirect and format the email nicely. The sooner this can be done the better.
This will be used on a GoDaddy VDS linux server.

Get my php form to send!
I have a finished html contact form and a php mailer script. The script only works for text fields and textareas, it cannot process user inputs on checkboxes. Ultimately, I just need someone to get the form to work. For someone with experience this should be really simple, most of the code is all there, just needs to be the right syntax.
