Simple Page Html Contact Form
Required skills: Website Design
We need to have a 1 page simple HTML contact form using email-form.com script. We have purchased the script and have installed on the server but need HTML page and contact form created. We have a PDF brochure that contains 90% of the information we want to ask with the online form. The form's option will be to save results to a XLS for spreadsheet and email to multiple emails with thank you page for more contact. All is possible through the installed script. Manuals available if needed.

Just to explain when the customer logs in with there ID and password they are redirected to there own page, example : www.timegeniehd.com/147852369 the number being there customer ID, I just need the redirect, I will create the customers web page

I'm looking for a freelancer to develop a really basic teaser page that displays some information and includes a contact form linked to a database accessible via CMS and emails the contact info to the administrator.
Contact Form Change
Our client's website contact form requires changes.
We are attaching current form (PHP Code) and the form to be replaced (HTML Code). So that the information is send to a Email Service Provider.
The website is built on PHP code. As such when we replace the form it need to be compatible.
