Custom Live Support Script

Hey guys,
I need a live support script that will be for members only. Basically, what it will do is be an icon in their user CP. When the user clicks on it- it brings them to a page where they can check 1 of 5 boxes (their problem)and then brought to a live chat with one of our operators. The operator admin (web based) is already 90% created, you'll just have to integrate it. Once they are with the operator, in the script will pull their information. This information will be displayed along with the problem that they're having (the box they checked at the beginning)
From there, I can go in and fix the problem.

One is an iframe based app script, but it lacks many of the features I want. If you can charge me less and add the features to the script, I would greatly appreciate that.
The second example is a fully functional script, but it is based on FBML, and it does not have the ability to create apps from admin panel.
I will test the script and make sure the apps are not deleted for at least 3 days before payment.
