Here's what I'm trying to accomplish:
I have a simple web form that emails customer signup info (name, address, email) with each submission.
My user then copies and pastes that info into her software to generate a mail merge
I want the form on the website (currently just an email form) to capture the data and write to two files when completed and submitted. Form submissions will ultimately need to go into .csv files.
One file should be called dailylcatalog200604101.csv (with sequential dates each day) and one to be called allcatalogrequests.csv that overwirtes each day.
dailylcatalog200604101.csv gets emailed automatically each day at 12:01am
allcatalogrequests.csv just recompiles and overwrites on the server each day.
Tthis solution may involve a MySQL database, or may not, whatever is easier.
This is just a "catalog request" form, and we simply want to have the daily file emailed once a day, so my user can merge into MSWorks or whatever they choose, and send out customized letters and catalogs & print labels.

I need to create a page with a form in my wordpress site (www.ipledu.com). This form should send the data to a database, such as using the "Contact Form 7 to Database Extension" (http://wordpress.org/extend/plugins/contact-form-7-to-database-extension/). The form should have around 6 fields and the possibility to upload a file.

A user login needs to be created- the user should only be able to see records for rows of data that have their user id. The admin needs to be able to create users and pause users from being able to login. The admin tool will also specify the ftp location to pull the csv file from for the user.

styling as in the PSD and be compatible with all major web browsers -
FF, IE, Chrome, Safari.
One of the form elements ("Mobile") is a SELECT box and so I believe from my limited knowledge may require using jQuery to style the selectbox.
This is a small, short project and I need it done within the next few hours.
The PSD file is attached so you can see what is required.

Second, I'd like the developer to install the plugin onto a WordPress site and use it for an assessment form which I've created in HTML. It is several pages long, about 24 sections long. It will need to be installed onto the SSL pages of the site.
