Required skills: Perl/CGI, Javascript
Need soneone that can take the basic form that I have now and can add several javascript functions to it.
I HAVE THESE SCRIPTS and want them added to the forms functions:
1 - Accept Terms or Form Acknowledgement Page
2 - Validate All Fields & Email Address
3 - Install Dictionary
4 - Smut Engine
5 - No HTML
6 - Email This Page To A Friend
7 - Submit Link - uses a link or image instead of a button.
I DO NOT have this script but want it on there -

Plus, I need to be able to call this script up via remote hosting.

i've created an 'id' tag for each option to let you know what value should populate the hidden field based on selection

convert some javascript to jquery
Convert a working javascript solution for loading images and showing/hiding some options into a jquery-based solution. All the functions are present, they must be rewritten in jquery.
