Create a Design in Photoshop; then Slice PSD to .ASPX

I created a web page template in MS Word that I want duplicated from scratch in Photoshop (see attached template 6). You should NOT use any of the Word images (only use text content from template 2 attached). I want you to create a new design in Photoshop based upon the design and layout attached. I've had designers in the past who copied images from Word into Photoshop and I declined the draft. You have to be experienced with the various tools in Photoshop and understand shading, gradients, layering, color combinations, etc and the subtleties of design. If yes, then this is your project! Although I do not need a lot of creativity in terms of creating a new design from scratch (you will duplicate the design attached) but you will not make it look good and make the web page propertional if you do not understand design and Photoshop.

B. Slice psd file into web page (HTML or .ASPX)
C. Create a Form using the above template where the fields are nested inside the template
D. Slice the 2nd page form
E. Make the Form work (I'm primarily looking for designers so if you cannot do the programming that is OK for I can find someone to do this)
Logo: use a stock image but change the size, color, and perhaps manipulate it some to fit the template
Colors: version1 (black background w/white active area) version 2 (white background w/your choice of active background)
Size: background is 100% but active area is centered and 980px wide (margin at the top and lots of background space at the bottom)
Picture: use the one I provide of guy named Ray Johnson (proportionally to fit the active text area)
High res images: 5 links (red circles top right) and 5 form buttons or circles that link to forms; logo should be high res
Text: use all the text content you see in the attachment
Slicing:
I want only a few images, picture, buttons, etc, but the majority I want to make sure the text is regular text and not an image, the company info, etc. I want the ability to change most of the text through the code without having to create new images. I want the ASPX version to as close to the PSD version as possible and I will be a huge stickler making sure the two are as close as possible.

There are bits and pieces which can be given and most of the job is actually create the PSD and conversion then. The design is a simple, minimalistic web 2.0 design for a social network.
