My profession is English Instructor.
I have PDF forms that I made on PDFPenPro.
These forms are actually Fill-in-the-blank tests.
When a students fills in the blanks there is a "submit" button.
I want the data from the blanks to enter into a database.
I need to be able to follow the instructions on these web pages:

So, I need a database and not only that ~ I want someone who is willing to teach me how this is done.

Auto-populate Pdf Form Fields Based On User Choices
I will need to be provided all scripting and/or table format used in order to accomplish this project so if any changes need to be made in the future I have all the information.
I have attached the orignal blank form.
Any finished product (auto-populating form) will be my property and no further distribution of this form will be allowed. Once the bid is accepted I will provide my custom form of this pdf so that the auto-populate features may be added.
Import Xml File To Pdf Form
this is very simple. BUT you need to have Adobe Acrobat Pro in order to do this task. I just need you to take my PDF form, open it in Acrobat Pro, then go to the Forms menu and click Import Data. You will import the data from an XML file that I send to you. I would do this myself, but I don't have Acrobat Pro. I need this done now!

Create Fillable PDF Form With Submission

PDF Form submit via website email results
ut and hit the submit button and the information will be submitted through my site, then sent to my email still in PDF format. HTML forms are out of the question unless you can make it look just like my original PDF and as long as the results still come in PDF form just like my original pdf.

Whenever a new invoice is creates, with each entry, this link button will appear and clicking on this will open a web page and that all we need. If possible we can post a predefined form to a web page or just open it.
