Ebook Design, Formatting and Validation

I have already created an ebook in Word/html of around 40 pages but have problems with formatting and the table of contents. I have created a cover.
I need someone to correct the formatting issues, make sure the table of contents is correct, add in intro to the book and basically make the book look professional. Then get it validated for use of all common devices including Kindle, Ipad, etc. I'm nearly there but I need some help with the finishing touches. No writing work required.
Around 20 books to follow after this one.
All necessary files attached.

Form Design And Validation

I prefer some type of jquery vs a pop-up java validation, and it needs in some cases to be more intelligent than just required or not required. It will need to to also validate field type.
I also have some APIs that I'd like to call to pre-populate some of the fields. Please let me know if you can do all of these things and if you can link me to a sample form validation that you've done that would be great.

Formatting and layout for a small ebook plus simple cover for same
to go along with a yoga CD that I recorded. The workbook is approximately 5000 words and will have up to 30 photos. I created the ebook in Word, and want someone to make it look professional, using photoshop or In Design etc. A couple of the photos may need to be altered (adjust the color) a little as well. The final version will be in color (for use on the computer), and then the book also needs to be saved in black and white so that it can be printed out as needed.
