Simple Lead Generation Website Needed

I am looking for a simple Lead Generation Website for local services.
*I want clients to fill out a detailed form including contact information.
*I want the form to be emailed to local service providers based on geography of the client (ex: if the client is in Atlanta and needs a plumber, an email will be sent to my list of Atlanta plumbers).
*I want the email to include all parts of the form except the clients contact information.
*I want the email to include a Paypal button or link that is generated specifically for that client submission.
*If the service provider wants the contact information they will pay for the lead via the Paypal link.
*After payment is made, the service provider will receive the client contact information (either by email or redirect from Paypal).
*Only one service provider can purchase the lead (if there are five plumbers, only the first one to purchase the lead via Paypal can view the lead contact information).
*I would need to be able to add service providers by region.
*The automated email will need to send emails to only service providers in that region (submitted by client in the form)
