this simple data typing project. and i want the freelancer to complete this propject in the minimum time level and with the good accuracy.

Boonex Dolphin Simple Data Entry Module 2

Boonex Dolphin Simple Data Entry Module
I need a really simple data entry module for Boonex Dolphin. This is really as simple as it gets. I need a module with 3 data entry text fields - Name, Email and Other Info. This data should be stored in a new table in the Dophin database when the user clicks the Save button. I should be able to add this module to a new page within the Dolphin framework. Thats pretty much it.

2. If some entries have fields that I have not specified, please add them as appropriate.
Should be a fairly simple data entry task that should only take an hour or two of manual work, particularly since it is a searchable PDF that you could copy/paste from/to. Probably not worth the time to write a script to parse it, but you could do that as well if you'd like.
