Correct simple .php / sql script bug

My site has a sql report generator which is not displaying the correct records.

1) select "Edit Your Collection"
There will be a list of 38 items on two pages.
2) select "Generate Reports" and then without making any selections, click on "Generate Report"
That report should have the same 38 items as appeared with the Edit option.
All of the items using the Edit option are correct. The report items are not correct. I would like to have the report script corrected.
PLEASE DO NOT make any changes to the collection list. If you would like to explore this database and how it works, you are welcome to create a free account at the site and explore it with your own account.
The script to be fixed is only 190 lines. This should be an easy one for somebody who would like 10 more stars in their feedback.

stores the cookie on the users computer, when the url includes a aid= - so aid=hello has to be stored as hello in the cookie - or aid=world has to be stored as world in the cookie. Cookie Epxiration days should can be configured.
The second code snippet is in php format and will do an mysql insert when there`s that cookie on the user`s computer.

Simple Php Sendmail Script Changes
g form of around 20 fields currently this is send via a .asp send mail script.
We have moved the site over to linux and now have a simple PHP sendmail script (replacing the ASP version), we need to amend this to send over all of the fields from the website.
I will provide the web page content and the sendmail script, it just needs someone who understands PHP to get it working.
Should be a simple and quick job for someone who knows PHP.

Each cookie can only do one mysql insert, after that it should be expired/deleted. If the user has more than one cookies, it also must make only one insert.

I need a simple php autoresponder script installed on my server. I have complete instructions for installing this script that are available if needed.

user visits my website enters email, and checkbox script checks for errors, if errors returns them to page with red color errors
if ok ...it writes the email to .txt file and send them to the confirmation page
this should take 1 hour or less and I need it done right now, as soon as you bid send me a yahoo instant message so I can give you the ftp info and discuss it.
