VB.Net development of desktop application with MS SQL server

What I need or require:
We need a VB.Net developer that can create a simple program to write to an online MS SQL database, and read from it. The information that will be written to the database includes a title and a date.
What I already have versus what the contractor will deliver:
I'd like the developer to create everything from scratch. It's a simple program so I don't expect it to be a huge effort. The online SQL hosting account will be provided.
Other context/requirements that contractors will need to know:
- The application should use the IE Internet Settings to connect to the online SQL hosting account (and still work in case a proxy is used to access the Internet location where the DB is stored).
- Structured, and simple application design. Don't focus on the GUI. What is important though is attention to sound programming practice. It must be object-oriented with comments that make the code easy to understand. Avoid using complicated API calls/3rd party DLLs.
Specific expertise/background that I am seeking:
Any proficient VB.Net developer who's had experience working with SQL servers. 3+ years experience.
Timeframe for delivery:
Need to have this running within 3 days of the winning proposal accept date.

I have finished creating the database and tables and have also created a dsn for sql server and have included it in all my asp files. But I am getting "server 500" error.

The compiled executable is 1.4Mb and contains everything for the application with the exception of some dll's it uses for some of the controls, its source code will be available, its written in c# and we use visual studio 2005 to edit.
Also conversion to VS2008 would be required too.
Internally my developers tell me there is about one days work to complete this but due to other projects and time constraints we can not do this internally.
