DHTML collapsible sections – single page
Either ONE page Javascript or AJAX technology
Javascript calculations
Currently the data is in an EXCEL Spreadsheet (SMALL section is included for illustration)

Purpose:
Collect data in a form (333 questions in 33 sections) using RADIO BUTTONS for each question
WEIGH and SCORE each answer (scores will be provided)
Send ANSWERED questions + SCORE to an email address (PHPMail)

HOW THE SCORE WORKS:
Each question has 4 radio buttons:
0:Not Applicable
1:Little
2:More
3:Very Much

The WEIGHT of the score is assigned per section:
Weight of J5 = 12
Weight of K8 = 4
I will supply a spreadsheet with the WEIGHT for each section (66 Sections)

EXAMPLE SCORE:
A question may affect different Sections depending on the WEIGHT of each Section:
Question 25 = is scored as follows: J5 = J5 + (Score*12) and K8 = K8 + (Score*4)
Question 26 = is scored as follows: F3 = F3+(Score*7) and J5 = J5 + (Score*12)
As you can see, the answer WEIGHT for J5 = 12 and the answer WEIGHT for K8 = 4 and the answer WEIGHT for F3=7

As you can see Question 25 affects the total of J5 and K8 and Question 26 ALSO affects J5 total.

END RESULT:
Create an EMAIL with:
TOTAL of each Section – Sorted Highest to Lowest: J8:45, A3:44, K2:39, A1:38, etc. Etc.
All ANSWERED questions score number Sorted by Body Part (discard NON-APPLICABLE answers)
i.e.
Bladder: Urinary Tract Infections = 2
Blood: High Blood Pressure = 3


User experience:
User sees page with a body and instructions (how to fill in the forms, there are 333 questions in 33 sections)
FIRST: Collect NAME, GENDER, AGE, EMAIL ADDRESS
>> Send an email Immediately subject: Clientname = filling in Questionnaire: Nutrition (Capture email address etc. If client completes or not)
Based on GENDER, SHOW ONLY QUESTIONS MARKED FOR Male or Female
Based on AGE, ADD Questions Marked CHILDREN for age UNDER 21

User cannot skip pages, must fill in pages in sequence.
Program allows for BACK and NEXT – shows progress bar. i.e. location in 33 steps

When user clicks NEXT, the previous section closes, and next section opens.

Each section shows NAME of body area: i.e. Eyes, with a line to the body image, and eyes area shaded (grey)
LAST PAGE: GENERAL – Please fill in any additional information or history we may need to know for your case: (Free form)

At the END of the questions, SHOW THE TOP SCORES as follows:
