Extend Existing PHP / MySQL Form Function
Required skills: HTML, Javascript, MySQL, PHP, Script Install
I have an existing form on my website, I want to extend this script so that it prints a PDF and emails it. It has a function that does some calculations based on a MySQL lookup.
I also want to add two new lookups/ calculations to the existing function.
It's easier than it sounds, especially for a seasoned PHP/MySQL programmer. You can do all the testing on your system, and I can send you the scripts and mySQL dumps needed to debug and edit and complete this.
The catch is that I want this done immediately. The PDF generation part is not as important to me as adding the two new sections. This work should only take an hour at most, so quote it that way, I am a systems engineer, so we should be able to knock this out fast.

Need New Features Added To Existing Php/mysql Script 2

- When I click "Canada" it does not open an email on ecommerce page.
- When I delete an item, the transaction info is lost.
- The totals do not calculate properly on the reports page.

1st uploaded thumbnail address will go under field imgname, 2nd under imgname2, etc.,
After submitting 'add record' from moodalbox it disappears & parent page refreshes | updates showing new record.
Moodalbox should be laidout & look very close to provided screenshot with different text fields of course.

rm on the page which will contain 2 dropdown boxes and a submit button.
The complicated part of this project is that the values entered in the URL for user selected values in box 2 on the form will CHANGE based on what the user selects in box 1.
*please note though that the visual/user values in box 2 do not need to change, only the values the form places in the URL for the next page change!
