Open Guest form one-page-checkout presta

Hi - I need help to make a small change on one-page-checkout in prestashop 1.4.2.5.
Please take a look at the attached picture. Instead of the "Checkout as guest" button, I would like
to show the form for guest checkout. (the form showed when pushed "checkout as guest" button.
Attached is image and order-opc-new-account.tpl and authentication.tpl

Joomla Travel Enquiry Form
website. Basically i want a link in the article (Enquire about this safari) linked to a form, when a user clicks on the link it can either open the form in a popup or open the form in a new page autopopulating the article title automatically.
The most important thing about the setup is that it must pick the page title (or itinerary) and send the info in the submitted form. The configuration should be done with RsForms, Fabrik or Breezing forms.

Create C# Form To Work Exactly Like A Microsoft Access Form

if you open form back up it is blank and if you open the report - now it is there- (report is based on table2)
Basically I have a access form and two tables and one report-
I use form to hold data- untill I want to submit data-
When data is submitted it is sent to second table and first table is empty.
I am not sure this can be done- but i want a C# form (.net 2.0) work just like my access form - and still use access database for tables and reports

Change 4 Forms To One

each form in properties- i have row source type - Value List and I have list of descriptions in Row Source.
In the future I will add more company names and descriptions- so I am not sure I should be using row source?
mainly I am hoping users to open 1 form and when they get to company name they choose a name. Then when they get to description field- they only see descriptions for that company to choose from.

Then click Submit. A new tab opens and you have to scroll to bottom, and hit submit again.
