I have a survey form generated by survey monkey. I need it to be laid out so we can do a printed version. I attach a copy of the survey for you to look at - we need the same lay out but with the following changes:
- formatted to take up fewer pages, but still keeping font size to 12
- addition of branding (colour, logo) - i can send you this info
- remove scroll boxes that appear in this version
