Simple Contact Form for Website
Required skills: HTML, PHP, Website Design
I'm looking for a smart freelancer with experience in php to create a simple contact form for a website of mine. I will provide you with the fields I want the form to include, the email address I would like the form to submit to as well as providing an example of what I would like the form to look like.
I also require that the form has captcha functionality to stop automated submissions.
Preferably in HTML email format/layout. Use of HTML/CSS/PHP is preferred. The code will be used on an already designed HTML website.
This is a $ 15 job and I would like it completed with 24 hours of accepting the job. Please do not bid if you do not accept these terms.

SIMPLE Contact form _SendEmail Request / Jquery / Ajax/ PHP
u must be able to take this form and place it into my existing jquery mobile website. I will give you all the files you need to test your PHP form with my mobile website. second is, that there is one simple css background fault, which i cant fix. if you are experienced in css, its a thing of seconds for you to fix it.

i have a small one-page html website where all menus are linked to external sites. Now i need a contact form which is linked to an internal site - a second page. The form should be emailed to a email-address. I have attach a screen of the form and how it should look.
The form should be based on php and should check for data entry.
It would be great to integrate also a captcha image.

t should change. The email and name fields should be on the left of the fields. The submit button should be on the left and not on the right.
