Required skills: MySQL, PHP, Script Install
Programmer needed for small php and MySql database project.
PHP Script needs to return data to a “results” web page and email from a database based upon website visitor survey answers. Script also needs to send the same data returned to the web page dynamically via email to the customer. Script will also need to store customer inputted data in database.
This project is a rush job and must be completed within 5 days. Must be proficient in PHP, MySql, and CSS. Looking to build long term relationship with many similar projects in the pipeline.

A piece of software will access this php script and check a mysql database for a valid user's email address and password then redirect to a successful page/file if a valid user is found and a message if one is not found.

also the script should have some security to check for a shared login maybe by ip, so if more than 2 ip's use the same login it will automatically ban that login information.

ose picture is being displayed. After submitting the answers, a page will be displayed where both the visitor's and the member's answers will be listed. Clicking a link will bring the visitor to a different picture with the same set of questions where the process is being repeated.
Please look to do this as cost-effective as possible. If there are scripts out there that you can modify to include all the functionality I need, feel free to do it so you don't need to start from scratch.
