Standalone Web-Based RMA System

We require a standalone web-based RMA system which allows customers to return items from our many avenues of sale, i.e OpenCart Shopping Cart, Amazon, eBay, Buy.com.
It would be preferable to be closely integrated with OpenCart (www.opencart.com) shopping cart.
It should have the option to file a new return, by either entering their OpenCart order number, last name and postcode (so it can look up their details) or for the other sales avenues (Amazon) storing details on their name, address, item, order number, reason for return, refund request etc.
Once they have filed the RMA is should generate a unique number for them and generate all the info into a form to be returned with the item. They should then have the opportunity to add the tracking number to the RMA claim once it has been posted back.
They should be able to check for the current status of the return, including an admin panel to update the status and add tracking numbers or refund confirmations. We are possibly looking for some kind of messaging system to be inbuilt as well. This can be discussed.

ce the data in a MYSQL db. Then move the txt file into an archive folder. It needs simple user management, 1 admin who can see all RMA records and the ability to create users who can only see there records. All information will be filled out on 1 form using javascript to determine possible choices based on certain conditions. No design work is needed, this is for private use and will not be open to the public. Please keep code clean and simple for easy updates later.

Looking for a programmer to design and program a standalone insurance quotation system (not internet based). We can show scren shots of a similar system we use. System will have 20 or so fields and once completed will return a premium (some liason may be also required with some insurance companies). Inputted information will need to be populated to a proposal form and information automatically email to insurer in an EDI type format.Ongoing maintenace and support will also be required.
