1 Page PSD to FULLY valid HTML/XHTML/CSS

1 Page PSD with form to be converted to valid, compliant HTML/XHTML/CSS. Please use very CLEAN markup, commenting where necessary. Page should utilize an external stylesheet. Please note the two buttons that have rollover effects/hover effects available in the PSD. Where available, all text should be TEXT not images, unless the font choice makes it impossible (in 2 buttons and in one area that has a text effect and in logo).
Form contains 6 fields.
You do NOT haver to do any of the form processing or scripting or validation. You ONLY have to convert this PSD to nice, clean CSS/HTML.
Project MUST be completed within 48 hours of acceptance (NOTE, NOT TWO BUSINESS DAYS BUT 48 REAL HOURS). NO LONGER.
Flat file attached with certain identifying information removed via white squares. Will send PSD to the chosen winner.

You can make the carousel code static for all categories and fade it in, we will make it dynamic.
We only use jQuery and it's plugins.
Anyone we work with on this will become of use to us in future projects as well.
