Electronic Forms (Survey Monkey + logic) conversion
Required skills: AJAX, HTML, MySQL, PHP, Ruby & Ruby on Rails
We are looking for an individual to take our SurveyMonkey survey and convert it into a custom form-based survey that will be hosted on our web site, with the backend being a MySQL database that can easily be exported to excel. The survey will require some page-skipping logic that SurveyMonkey currently does not support. The 'subject line' for a successful bid must say, survey-monkey conversion, in order to be considered. We are flexible with the programming language(s) used, as long as it delivers the needed results. This may include CSS/HTML, PHP, Ajax, Ruby on Rails, etc. Finally it must provide email notification once a survey is complete.
Success bids should be in the $ 50 range.

Convert Auto dealership to 100% electronic signature pads
to accomplish this and open to suggestions, but the customer information exists in a software program call contact managment and I can export the customer information to excel. I am thinking of having the sales reps export the file to excel open it, and run a macro, and that macro will be the start of the electronic signature process. I need the ability to add and delete forms as new forms arise.
