Hi. I would like somebody to make me a lead capture form for Facebook.

It will contain some text and graphics (which I will provide) and then ask for up to 10 piece of information.

When they hit send, I want the data to go to my email.

I don't want them to be redirected off from Facebook, so upon form submission, a new window should appear that thanks them and has a link to my own website.
