We require a simple website for separate users to login, create (edit & print) rental contracts / tenancy agreements.
The documents are to be generated by a web-form, once completed and submitted, results are saved to MySQL database. Then the data can be merged into an existing document / template and display as a PDF for printing / saving.
When new documents are created / entered, the data needs to be written to a MySQL database. No design work is required, using a simple layout we provide, you are to create the features in html / php & MySQL.
We need: Login page to access website, then simple admin interface…

• I need to view all User’s saved agreements (drop down for all, or each user’s name). Only parent has option to ‘delete’ any contract…
(Notes: Document is a tenancy agreement / rental contract. Data is the typical fields such as dates, rent figure, deposit figure, landlords details, tenants details (static data), also a some queries such as pets = yes or no, swimming pool = yes or no, would dictate which clause is used in the contract, etc. Current version is only 4 pages of A4 in total.

PHP page to edit mysql db
We have a small database with 2 tables.
Approximately 20 rows per table
We currently use sqlyog to create/delete the rows in the table.
Instead we simply want a php page to do this. i.e. be able to edit the mysql db
Note: The php/mysql is hosted on windows not unix

custom aMember plugin to integrate additional mysql db
automatically populate new mysql db with user info (username, fname, lname).
When a member is deleted from the amember db, the integration plugin needs to delete that user's record from the plugin db.
That's it, pretty simple!

Forms That Write To Csv
I have a site built and I have created 20 forms.
I would like these forms to write to 20 separate excel files (or I believe it is easier to write to CSV file)
Each time someone submits the form I would like it to add to the CSV file and compile the list of sumbmissions and also email a recipient.
To explain details further: There are 10 regions to the site and there are two forms each region. Each region has two registration forms.
