Pet Shelter web database with image and scan upload

I need to have a simple web front end for a database I am providing to the local pet shelter.
I have a database with approximately 20 fields
I need to be able to connect to a mysql database that I provide
be able to add/delete/scroll through the records
be able to upload a picture from a file location or camera
be able to scan in from a TWAIN driver source ( if I click on scan, it loads the TWAIN driver and scans into the database and you can see the image thumbnail.
be able to print the whole report (report page + attachements from any web page
be able to show a thumbnail image of the pet in the upper right page
have web based navigation buttons for forward/backward scrolling
be able to search on a pet name or acquisition date, or acquisition location
And to help me upload the database to my server (pretty sure I wont have any trouble.
Milestone payments: 1/3 down 1/3 to show me the app running on your database 1/3 when you send me the database and its tested on my server

Populate Database with Users and Projects
bjective is to populate the site's database with users and projects, so that it does not look empty upon site launch. The client already has about 100 projects that need to be entered, but they would also like to add projects from other sites to populate the rest of the categories.
This can be done by either manual data entry, or through the development of a spider script to grab project information from other sites. The lowest cost will be selected.
