HTML5 mobile version of php+mysql web application

Hello,
We are looking for an experienced HTML5 developer to convert our php-mysql web application into a HTML5-CSS universal mobile friendly application. We want our clients being able to enter data and, save and print regardless the mobile handsets (Iphone, Palm, Blackberry, Android, etc ) they are using. Must work in both online and offline modes.
Client must be able to
1) login with user name and password
2) select a few variables
3) enter and save data (numeric+text)
4)insert and save physical activity logs
5) generate and print spreadsheets and PDFs, all via the handset (if possible)
Current web application is developed in php – mysql. All options are currently pulled from Mysql database using php code. We will provide all required php code and queries used to retrieve data.
Please give your best price estimate and examples of similar previous works.
