I have a wordpress theme that needs a custom header. The header of the wordpress will be exactly like the one you see on this site:

Also, i am attaching an image of the section that will be needed.
Please make sure the header will have the sites (http://www.patientsurvey.com/) navigation. Plus, the wordpress theme's own navigation. You may have to tinker with the colors of the theme to make sure it fits in nicely with the header.
please dont send me your 5 page copy / paste project list. send me relevant questions/ comments only.

ject), and since the last guy I had who did this kind of thing for me quit freelance and went to work for a company, I'll need someone in the future to do other stuff like this. I'm just hoping whoever does this job is willing to other small jobs in the $ 20-$ 50 range (instead of doing one job cheap like this and then charging a lot more in the future). So if you can do work like this consistently cheap, that's what I'm looking for :)
