Wordpress blog needs small changes

Hello,
my blog www.blogalberghi.it needs the following small changes:
In the homepage there are now three boxes:
1) The first one is Confronta Portali. I need you to create the following box
Please view the attached document for image.
The calendar should be the one of the web site

When the user insert the values and click GO you have to redirect to:
http://confronta.blogalberghi.it/SearchResults.aspx?fileName=<city>&checkin=<check-in>&checkout=<check-out>&languageCode=IT&brandId=null&Adults=<ospiti>&Rooms=<camere>
The first link Cerca per Nome Albergo should go to http://confronta.blogalberghi.it/HotelNameSearch.aspx
The second link Sofglia per Paese http://confronta.blogalberghi.it/BrowseByCountry.aspx
2) The second Box Confronta Alberghi should work like the Third box I’m describing below.
3) The third Box Confronta Ebay has a textbox it should work as follows:
1. The user insert a City and when the Cerca button is pressed
2. There should be a php file where I can redirect the different cities for example:
a. The user types Roma I will redirect to a page I can configure.
b. There should be a php SWITCH I can configure.

4) When I go to a post there should be on the Top the Path:

5) The upper border of the footer box is bigger then all the other borders, should be the same.
6) There is caching in the blog, but the first page load in 5 seconds (too much) you have to implement all the suggestions given by Yslow

My Wordpress blog needs title, description and meta tags placed on at least 20 pages as well as robots.txt functionality. Only Wordpress SEO specialists with several testimonials please.
