PHP Form adjustments
Required skills: PHP, SQL
We have one php form that has 7 tabs within the form. All of the data from the form is captured in tables (we have a MySql database).
The adjustments needed to the form is as follows:-
5 x text fields need length increased to accommodate entered data
1 x drop down needs the length decreased
1 x number only text box needs the length decreased
5 lines of (text only) instructions need to be removed from 5 separate questions
All of the following need the data to be captured by the table
1 x text box needs to be added
2 x check box (or radio button) need to be added
3 x ‘upload’ and ‘submit’ field (for jpeg / picture files)
3 x line of text for 3 x separate questions need to be added
1 x check box needs to be changed to an on-click response with 4 further questions
1 x check box needs to be changed to an on-click response with 2 further questions
3 x check boxes need to be changed to multi select drop downs with brief instructions for user (i.e. to use ctrl when selecting). Each drop down will also trigger an on-click response with a further question. If the on-click response is triggered, the data needs to be stored in the table and also update the member profile
1 x drop down that will trigger an on-click response. The number of on-click responses will depend on the number selected in the drop down. The numbers in the drop down need to be increased to 20.
E.g. if 5 is selected in the drop down, there needs to be 5 sets of questions with 2 text fields in each. The data from the text fields needs to be captured in the table.
1 x further drop down that based on a ‘yes’ response will trigger another on-click response beside the additional questions above (a number only field)
14 check boxes need to be changed to on-click responses
In addition to these, there are small alignment issues that need to be done and small tweaks such as the cursor automatically moving to the next field, and changing some check boxes to radio buttons.

Php Form - Need Sessions Added
have a PHP form that has been created for me. It has 11 or so steps, and posts to an SQL database.
However, there are no sessions so half way through the form you can't go back and forth.
I need someone to edit this form for me. I'll pick a programmer in the next hour and ideally it would be great if we could get this done in 2 hours or so.
I'm in the UK so I would prefer a UK programmer as the time difference doesn't suit.

Simple Excel To Php Form

Modify Php Form Script To Cloak Emails
