Filipino Programmers - Site Like SurveyMonkey.com.

Hello,
I need a script that will do exactly what surveymonkey.com does with some additional features.
All the features are listed in the pdf document attached to this project.
The script has partially been created already by the previous programmer, who decided to abandon the project, because the team that was working on it quit working for him.
(some database work has been done and the question types have been created)
I have the sourcecode and all the files that have been completed. That will be sent to the winning bidder.
Due to that experience, no escrow or advance payment will be made for this project. You can keep everything on your server and transfer the script to me when the payment is received.
I want a good looking, nice interface as well.
You will also be asked to provide design for this as well.
Please provide examples of your previous work.
You will be asked to communicate daily with me through Yahoo Messenger to report on the progress of the work.
Lowest bidder will get the job. I won't pay more than $ 250 for this project.

easy task for people with the right knowledge!
