Need someone to create web forms for a PostGre database:
1. I will supply list of fields (approx 50) - you create a PostGre table - upload on our server
2. Create a PHP web front-end form for database based upon a Powerpoint mockup
3. Create a report form based upon Word mockup
4. Create the ability to click a button on form to upload a PDF
5. Create the ability to log in with user name and password
6. Create the ability to log in with user name and password and search for a person or ID number
7. Create the ability to view data relative to that record
4. Upload on our server and test.

for a % of the company income, for long term consulting status. I'm thinking 25% of profits. This company would take some time to get 'off the ground' and the programmer must be patient. I would give a written contract specifying percentages, and conditions.
