Re-design Skin, Logo and content- opensource free script vid

Hi All,
I need a very simple $ 400 project completed for an opensource free script, from clip-bucket.com, similar to youtube. The following will apply to this project:
1. Download files from this link http://clip-bucket.com/download/file/ClipBucket-172-FINAL/9
2. Design a logo for netball.tv (need two (2) examples with unlimited revisions) needs to resemble netball.
2. Re-design skin and total feel of site so that it suits my domain name netball.tv. (need two examples with unlimited revisions). Needs to tie in with logo and resemble netball.
3. Add content to the following pages (content needs to be approved by myself)
- Add Categories
- Add Channels
- Help
- Privacy
- Terms of Use
I do not expect alot of content, just enough to run site.
4. Upload all files to my server and intergrate with hosting account (cirtexhosting.com), which I already have.
5. When completed, netball.tv will be fully operational and I will be able to manage myself.

Also need to add a twitter icon on top right hand of page pointing to @netball.

This is a logo for a COSTUME STORE. The name of the store is SUPER COSTUMES in spanish which is SuperDisfraces.
